Transitions Behavioral Health Programs
Patients with needs somewhere in between inpatient admission and true outpatient care
can find the help they need in our Transitions Outpatient Programs. These services include:
Senior Partial Hospitalization Program (Age 65 and over):
Monday through Friday: 10:00 a.m. to 2:30 p.m.
Provides Support and treatment for older adults by providing medical monitoring, case management, and therapeutic groups and activities. This program works well for patients as a step-down from inpatient treatment.
*Lunch included and Transportation provided inside I-285.
Adult Intensive Outpatient Counseling Program:
Monday, Wednesday and Thursday - Day Program: 11 a.m. to 2:15 p.m., Evening Program: 5:30 p.m. to 8:30 p.m.
Primarily focused on working adults who need scheduling flexibility in their care regime. The program is anchored in group therapy along with medication management, case management, and individual and family counseling, as needed.
Young Adult Intensive Outpatient Counseling Program (Ages 18 - 28):
Monday, Wednesday and Thursday Day Program: 5:30 p.m. to 8:00 p.m.
Primarily focused on young adults who need scheduling flexibility in their care regime. The program is anchored in group therapy along with medication management, case management, and individual and family counseling, as needed.
60+ Men's Support Group's:
2nd and 4th Thursday of every month, 4:00 pm - 5:00 pm
The group's main goal is to provide a safe, comfortable, nonjudgmental forum that advances the mental health wellness of senior men through the sharing of experiences, from grief and loss to hopes and celebrations.
